Mobile Crisis Worker

OneCare Crisis Network is now hiring Mobile Crisis Management Crisis Workers. The Mobile Crisis Management Crisis Worker will provide crisis management services and shall demonstrate competencies in crisis response and crisis prevention.

Duties:

  • Assesses individual client functional skills, behavioral health, and symptoms in the community.

  • Develops individualized client crisis and discharge coordination plans:

  • Coordinates individuals and families to treatment services

  • Services may include behavioral health, medical, and addiction treatment.

  • Performs care coordination duties.

  • Provides crisis assessment and intervention services in support of consumers including after-hours duties.

  • Provides coordination in support and management

  • Collaborates with other team members and community partners

  • Other duties as assigned by the Supervisor or Mobile Crisis Clinical Lead

Minimum Requirements:

  • A minimum of two year’s experience in providing crisis management services in the following settings: assertive outreach, assertive community treatment, emergency department or other service providing 24 hours a day, 7 days a week, response in emergent or urgent situations, attend Mobile Crisis Worker Certification Training Program offered by a partner agency.

  • They have proven strong client engagement skills.

  • Proven success collaborating with teams at various levels of care and with community partners.

  • They have demonstrated capacity to individualize care and tailor services to individuals at varying stages of change.

  • Compassion towards individuals with Mental Health Crises, and co-occurring medical and mental health needs.

  • Experience working with diverse communities preferred.

  • Experience using EHR for documenting services provided

  • Must have the ability to get along with co-workers.

  • Ability to meet strict guidelines with the documentation process

  • Must have a valid driver’s license, and reliable transportation.

  • Skills and Knowledge: Requires a working knowledge of well-developed human relations, interviewing, and writing skills; along with interpersonal sensitivity and awareness of client situations and values. Working knowledge of community behavioral health and/or addiction concepts, case management, treatment services, environment assessment, behavioral health disorders, and community resources.
